Health Caseworker
Minden Medical Center
Minden Medical Center in Minden, LA is seeking a PRN Licensed Social Worker to support patients, families and the behavioral health team. You will provide compassionate counseling, advocacy and discharge planning within an onsite facility, helping to coordinate care across disciplines.
The role emphasizes intake assessments, individualized care plans, collaboration in rounds, thorough charting and connecting patients with community resources. A current Louisiana LSW license is required.
