In this inpatient role, you will be part of a collaborative and supportive clinical team focused on delivering high-quality, patient-centered care. The setting includes adult and geriatric patients with a variety of conditions, which may include orthopedic, neurological, post-surgical, and medically complex diagnoses. You will work closely with physical therapists, speech-language pathologists, nurses, physicians, and case management to coordinate comprehensive care plans that promote optimal functional recovery and safe discharge. A typical day includes performing thorough evaluations, developing individualized treatment plans, and implementing evidence-based interventions to help patients regain independence in activities of daily living, mobility, and cognition. Responsibilities may include functional training, adaptive equipment recommendations, home and discharge planning, patient and family education, and coordination with the interdisciplinary team during daily rounds. You will contribute to goal setting, participation in care conferences, and timely documentation within the electronic medical record. Patient caseload often includes a mix of acute and subacute-level patients, with a focus on functional outcomes and safe transitions of care.
