Agency Website: VDH.Independently provides professional nursing care to patients in clinics, homes, and schools, and other community or congregate care settings. Performs nursing assessments, lab tests, takes measurements, develops and implements care plans, including administration of medication and case management. Participates in public health emergencies and community health assessments. Actions performed at this level require licensure as an RN by state code. Communicates verbally and in writing, analyzes problems, situations, and suggests solutions, demonstrates leadership, follows directions, and establishes priorities.

Skills in physical examination, health assessment, interviewing, and counseling techniques.

Knowledge of public health concepts, principles of public health nursing, principles of epidemiology, immunizations, basic nursing science, medical terminology, and case management.

May travel; weekend and/or evening hours may be required.

Additional Performance Management (if supervising)

Serves in a managerial/supervisory capacity and conducts performance management activities for staff.

Ensures relevant training and workplace safety for staff and conducts appropriate performance monitoring and appraisals.

Uses the nursing process to assess, develop/implement care plans, and evaluate outcomes. Uses the Quad Council’s Core Community/Public Health Nursing Competencies as an evidence‑based platform for practice.

Recognizes diversity of individuals/populations and applies appropriate language and communication tools, including cultural, socio‑economic, sexual orientation, and gender identity considerations.

Adheres to nursing directives, agency protocols and guidelines.

Practices within the realm of the Nurse Practice Act and related health laws.

Assesses health status and health literacy of individuals and families, including determinants of health, using multiple data sources.

Applies ethical, legal, and policy guidelines in the collection, maintenance, use, and dissemination of data.

Uses information technology effectively to collect, analyze, store, and retrieve data related to public health nursing services for individuals, families, and groups.

Employs evidence‑based or promising practices from across disciplines to promote health in communities and populations.

Accesses public health information for individuals, families, and groups.

Uses assessments, develops plans, implements, and evaluates interventions for public health services.

May administer routine child and adult immunizations to clients.

Demonstrates ethical standards of practice in all aspects of public health and public health nursing.

Uses data, evidence, and information technology to understand the impact of determinants of health on individuals, families, and groups.

Assists with referral and follow‑up processes to access community resources or services.

Uses research to inform public health nursing practice.

Models public health science skills when working with individuals, families, and groups.

Direct Patient Care

Provides competent, high‑quality, and safe nursing/case management in clinic, community, home, and other settings in a confidential, customer‑oriented manner that is culturally and linguistically appropriate.

Uses the nursing process to assess, develop/implement care plans, and evaluate outcomes.

Teaches and counsels clients and families about health promotion and disease prevention.

Selects variables that measure health and public health conditions.

Performs duties including communicable disease investigation, fit testing for an N95 or other respirator, donning/doffing PPE, supporting emergency shelters, and other related tasks.

Additional study in health promotion or public health preferred.

Experience in a public health setting or related disease prevention and health promotion.
