Schedule: 40 Hours/Week (5x8s)

Key Responsibilities
Conduct comprehensive patient assessments and develop individualized plans of care for Home Health and Hospice patients.
Provide skilled nursing services, including disease management, wound care, medication administration, injections, and IV therapy.
Monitor patient progress, evaluate treatment outcomes, and update care plans as needed.
Educate patients and family members regarding diagnoses, medications, treatments, and self-care management.
Coordinate care with physicians, therapists, social workers, and other healthcare professionals to ensure continuity of care.
Accurately document patient assessments, interventions, and outcomes in electronic medical records (EMR).
Deliver compassionate end-of-life and palliative care support when applicable.
Maintain compliance with agency policies, nursing standards, and regulatory requirements.
Demonstrate flexibility and adaptability in response to changing patient needs and schedules.
Travel to patient homes within the assigned territory while maintaining professional and patient-centered care.
Requirements

• Active New Hampshire RN License or Compact License
• Active BLS Certification (AHA)
• Minimum 18 months of recent Home Health experience (Required)
• Previous Hospice experience (Preferred)
• 2–5 years of Medical-Surgical experience (Preferred)
• Ability to work independently with minimal supervision
• Strong assessment, critical thinking, and communication skills
• Computer proficiency and experience with EMR documentation
• Valid Driver's License and Current Auto Insurance
• Ability to provide care in diverse home environments and patient populations
