Provides safe and appropriate pharmacy services to patients in the alternate care setting in accordance with policies and procedures, professional standards, and applicable regulatory and accrediting entities. Assesses appropriateness of therapy, dispenses medications, supplies and medical equipment, documents patient progress toward goals, intervenes as clinically appropriate and provides education regarding appropriate medication administration and use. Additional dimensions may include working with operating budget, revenue responsibility, purchasing responsibility, etc.
Directly accountable and reports to the Pharmacy Manager or Director of Pharmacy or Operations based on branch leadership.
Demonstrates ethical pharmacy practice and ensures compliance with all company policies and procedures, state and federal pharmacy rules and regulations, professional practice standards, and accrediting entities.
- Ensures the patient database is complete and compiles the necessary information needed to design a patient-specific pharmacy plan of care with goals for therapy.
- Assures documents that require the prescriber's or pharmacist's signature for billing are generated and obtained.
- Assesses laboratory results and performs pharmacokinetic dosing.
- Makes appropriate dosing recommendations and documents.
- Performs accurate prescription order entry to ensure medications are properly concentrated, without incompatibilities, stable and properly stored.
- Accurately submits electronic claims for medications, as applicable.
- Documents all communication with physicians, nurses, dietitians, patients and caregivers in the patient's medical record.
- Documents initial patient or caregiver discussion, education and counseling regarding prescribed therapy, medication use and information, infusion method, and after hours contact information.
- Thoroughly and objectively documents the patient's course of therapy, compliance with therapy, laboratory results, and progress towards goals of therapy with therapy-appropriate frequency.
- Ensures all compounding, clean room and anteroom activities adhere to all state and federal regulations, USP standards and company policies and procedures by all pharmacy staff.
- Ensures required documentation is completed on QA/QC records and compounding documents.
- Accurately maintains calendar for laboratory tests, compounding, delivery, and documentation updates
- Oversees the work of pharmacy support personnel and ensures their job duties are completed in a timely manner.
- Participates in the on-call process and documents on call activities in the patient medical record, if appropriate, and communicates with relevant personnel.
- Assists in the training and supervision of pharmacy support personnel, pharmacy interns and pharmacy-profession students.
- May assume role in of the Pharmacist in Charge or general branch manager during temporary absences and/or vacancies as appropriate.
- Moderate computer skills including MS Office, Windows Outlook, and Internet.
Moderate level of interpersonal skills to interact effectively and professionally with both internal and external customers.
Moderate mathematical skills to perform pharmaceutical calculations.
General knowledge of modern office procedures, filing, telephone techniques and office equipment.
Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Master's of Science degree in Pharmacy desirable) from an accredited college of pharmacy.
Currently licensed as a registered pharmacist in state of employment.
Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
Must be flexible with work schedule.
Dependable in attendance and job performance.
