New Hope, MN 55428Providence Place3720 23rd Ave S, Minneapolis, MN 55407What You Can Expect:In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. LPNs will work in conjunction with dialysis RNs to provide dialysis care.You will work 3-4 days a week - with possible travel to cover other SNF clinics in your region (per patient/clinic needs) and also be required to work alternating SaturdaysYour impact on the lives of our patients and their families will be felt every day.Patient Care: Spending the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment.Complexity: DSD Dialysis care is a complex but fulfilling service line. Autonomy and Critical Thinking: Provide comprehensive patient care in an independent setting. You apply your "core" skills to monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup. There's a lot to track and manage and the joy comes in witnessing how your care impacts patients' lives.Your training will include a combination of classroom and hands-on learning, through DaVita's award-winning training programsTraining may take place in a facility or a training clinic with a class of new teammates other than your assigned home clinicMay be asked to support other local SNF densWhat You Can Expect:Deliver care to patients who are often dealing with multiple co-morbidities which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge.Requirements:Current LPN-LVN license in the state of practice requiredCurrent CPR certification requiredIV certification required if required by state regulationsHigh school diploma or equivalent requiredMinimum of one year's experience in a hospital or clinical setting preferredBasic computer skills and proficiency in MS Word and Outlook required; functional proficiency in computer systems of DaVita within 90 days of hire required)DaVita is a clinical leader!
