Title: PRN Home Health RN OASIS Admissions and Recerts
Pay: Regular visit $45 to $50 SOC $100 to $120 ROC $90 to $100 Discharge $50 to $60
Employment Type: PRN, Part-Time

Responsibilities

Complete OASIS Start of Care, Resumption of Care, Recerts, and Discharges
Perform head-to-toe assessments and create individualized plans of care
Teach patients and families on meds, safety, and disease management
Coordinate with physicians and office clinicians for timely updates
Document accurately in WellSky Kinnser and meet compliance standards
Qualifications
Required
Active Texas RN license in good standing
Minimum 1 year recent home health RN experience with OASIS
Proficiency in WellSky Kinnser
Reliable transportation and valid drivers license
Strong communication, organization, and time management Preferred

BSN

Bilingual Spanish or prior wound care experience
