Responsibilities include collecting and reporting patient data, reinforcing education, supporting care plans, and participating in quality improvement.
Collects and documents objective and subjective data; reports changes in condition to the RN or provider for further assessment.
Contributes to patient assessments and supports implementation of the plan of care under RN supervision.
Provides developmentally appropriate care and evaluates outcomes of interventions, communicating results to the care team.
Participates in quality and process improvement initiatives.
Supports personal and professional growth through self-directed learning and unit-based activities.
Maintains professional boundaries and collaborates effectively within the multidisciplinary care team.
High School diploma or GED and completion of practical nursing certification or program

Minimum Experience Required:One (1) year of LPN experience

Hourly Range: $25.95 to $38.93
