REGISTERED NURSE

The Jupiter, Florida market is in need of a Home Health skilled RN-with Oasis experience we can do this several ways: REPORTS TO: Director of Nursing

JOB SUMMARY:

Professional member of home health team who provides skilled nursing visits in patient's home under the direction of plan of treatment established with physician and input from patient/care giver.

DUTIES AND RESPONSIBILITIES:

Provides initial and on-going assessment of client needs using the OASIS data elements incorporated into the comprehensive assessment
With input from patient/care giver and in conjunction with physician, formulates and implements plan of care
Evaluate effectiveness of care plan and make necessary adjustments
Provides for the emotional and physical comfort and safety of client taking into consideration their rights and cultural background
Receives and transcribes physician orders
Notifies physician and agency supervisor of unusual reactions and/or changes in client's condition
Documents all appropriate observations and treatments in keeping with agency policies and procedures
Participates in case conferences, team meetings, staff meetings, and performance improvement activities as assigned
Provide any skilled nursing service for which appropriately trained which is prescribed under plan of care. Prepares clinical progress notes as per agency policies and procedures
Provide 60 day summary of skilled services and client outcomes to physician and agency supervisor in keeping with agency policies and procedures.
Adheres to all agency policies and procedures including but not limited to the HIPAA privacy rule.
Maintains strict confidentiality of all patient, employee and agency information
Other duties as assigned by supervisor and for which he/she is qualified
Conducts self in professional manner at all times and in all situations
Participates in all agency in-service programs

QUALIFICATIONS:

Must be a graduate of an approved school of nursing
Currently licensed in the state of the HHA
Direct Hire sign on or relocation fees and sign on of $5000 after 90 days (FT includes phone and internet allowance of $50 each per month, tablet for documentation, mileage of .50/mile) Start date asap
