LPN – Home Health
Contract Length: 13 weeks
Current nursing license in the practicing state
Two years of experience as an LPN in a clinical setting, preferably in a home health or hospice setting
Proficiency in using electronic medical charting and time-keeping software (Homecare Homebase)
Provide nursing services under the direction of a registered nurse, assessing patients' physical, psycho-social, and environmental needs.
Assist in planning, coordinating, and implementing patient care in collaboration with the RN.
Communicate effectively with patients, families, and healthcare professionals and document care outcomes appropriately.
Educate patients and families on self-care techniques and assess their responses to care.
Maintain infection control measures to ensure safety for both staff and patients.
Assist with specialized duties related to end-of-life care for hospice patients.
Must participate in annual in-service and performance improvement programs.
Reliable transportation and current auto liability insurance coverage are required.
Must read, write, and speak fluent English.
