Nurse Practitioner (Full-Time) – Post-Acute / SNF Focus Greater Chicago Area
As a rapidly growing, compliance focused IV therapy organization, we are redefining hydration and clinical outcomes in the skilled nursing and post-acute space, empowering wellness and elevating lives, one patient at a time. We are seeking an experienced Nurse Practitioner (NP) to join us as a foundational clinical hire in the Greater Chicago market. This is a unique, early-stage opportunity to help build and shape a consultative clinical model focused on hydration health in skilled nursing facilities, working closely with both Corporate and Clinical Leadership. Opportunity to help shape clinical protocols and care delivery models
Serve as a consultative provider focused on renal and hydrational health focused care
Partner with Corporate and Clinical Leadership to develop protocols, workflows, and standards of care
Support program development in an early-stage environment where structure is still evolving
Build strong relationships with facility staff, administrators, and interdisciplinary teams
Deliver high-quality, patient-centered care in skilled nursing and post-acute facilities
Ensure compliance with all regulatory and clinical standards
Travel locally within the Greater Chicago market as needed
Active Nurse Practitioner license in Illinois
Strong experience in skilled nursing or post-acute care required
Excellent interpersonal and relationship-building skills
Comfortable in a startup or early-stage environment with evolving structure
Passion for building, improving, and scaling clinical programs
