Advanced Licensed Practical Nurse (LPN) - Telehealth

Your Role: Licensed Practical Nurse (LPN)
As a licensed practical nurse under the nurse manager's or designee's direction, the nurse provides professional and skilled nursing care for individuals with developmental disabilities in the office, homes and community settings. You’ll encourage independence, promote engagement, and help clients achieve their personal goals with compassion and integrity.

Provide direct nursing care under RN supervision.

Perform G‑tube feedings, flushes, and site care; Perform wound care and dressing changes per care plan.

Provide nursing respite.

Assist with toileting, bowel and bladder routines, and hygiene as needed.

Observe and report changes in condition to the RN.

Reinforce caregiver education on tube feeding, diet, and basic diabetes care.

Document care provided and patient response.

Chart behavior or condition observations on the computer.

Drive or assist the driver in safely transporting individuals with disabilities, ensuring comfort and support during transit.

Must be 18+ years old.

Active LPN license.

Home health experience preferred.

Experience with diabetes and enteral feeding preferred.

Comfortable with telehealth.

CPR and First Aid Certification.

Clean Level One Fingerprint Clearance Card (or ability to obtain).

Reliable transportation and willingness to travel locally.

Experience working with individuals with developmental or intellectual disabilities.
