The Role As a Licensed Practical Nurse (LPN), you will collaborate closely with our Nurse Practitioners. Your primary responsibility will be to provide holistic, high-touch, and coordinated care for a panel of senior patients, ensuring an exceptional patient experience.

Day-to-Day Responsibilities Deliver outstanding clinical care and support functions related to patient care activities

Actively engage and manage patient panels in collaboration with the Nurse Practitioner, extending the delivery of high-touch, quality care

Ensure efficient clinic flow by rooming patients promptly and assisting the provider in maintaining a timely schedule

Support post-visit activities, including facilitating referral completion, patient care coordination, and conducting follow-up outreach to patients as needed

Promote clinical and safety standards to drive high-quality care for our patients

Under the direction of the Provider, offers LPN appropriate “visits”, such as administering vaccinations and performing procedures, including but not limited to: ABI, spirometry, EKGs, etc.

Oversees the laboratory and related activities including reviewing lab orders, drawing blood, collecting samples, and preparing specimens for laboratory shipment or analysis

Prepare, clean, and maintain instruments and equipment in the exam room and laboratory

Experience and Qualifications High School Diploma or GED equivalent

Graduate of an accredited Licensed Practical Nursing program

Valid LPN license

3+ years of experience as a Licensed Practical Nurse

Primary Care experience is a plus

1+ year of Phlebotomy experience (preferred)
