Caseworker (Full Time)

We are seeking a full time Social Worker to join our team. The ideal candidate will have experience in a long term care or sub‑acute setting and must have previous healthcare experience. WiIlling to train the right candidate.
Preference given to candidates with nursing experience (CNA, LPN, RN)
Duties include:
Identifying individual social and emotional needs
Developing and Maintaining individualized care plans
Participating in the planning of the resident’s admission and discharge to the community
Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
Assess residents on a regular basis
Coordinate investigations for resident grievances
Requirements
Bilingual (Preferred)
Experience with MDS 3.0
Proficient in discharge planning and memory support
Long term care/sub‑acute experience required
Familiarity with EMR and basic computer programs
