Dialysis Nurse - Night Shift

Overview Sign On Bonus for New Hires . Paid one on one training and education provided. Empower, engage and train dialysis patients on peritoneal and home hemodialysis using state‑of‑the‑art training programs and equipment. Primarily works in the clinic setting and conducts initial home visits with another staff member present. Position functions as an onsite Patient Educator and Case Manager for patients with various shifts and work hours.

Key Responsibilities Accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients.

May delegate appropriate tasks to direct patient care staff including RNs, LVN/LPNs and Patient Care Technicians.

Participate in decision‑making, teaching, leadership functions and quality improvement activities that enhance patient care outcomes and facility operations.

Perform ongoing, systematic collection and analysis of dialysis data for assigned patients; document in the patient medical record; make adjustments or modifications to the treatment plan as indicated and notify Supervisors or physicians as needed.

Assess, collaborate and document patient/family learning needs to provide initial and ongoing education.

Direct and provide all aspects of the provision of safe and effective delivery of dialysis therapies to assigned patients in collaboration with the patient, home care partner, direct and ancillary staff.

Initiate or assist with emergency response measures.

Ensure correct laboratory collection, processing and shipping procedures are performed and reschedule missed or insufficient laboratory collections.

Identify expected outcomes, document and update the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.

Assist in the identification, evaluation, selection and education of Home Dialysis candidates and Home Partners.

Perform assessment and identify barriers of the Home Dialysis candidate’s home environment and partner/family readiness and ability to perform dialysis treatments in the home.

Train home dialysis patients and/or Home Partners on the safe, effective operation and maintenance of all Home Dialysis equipment and treatment supplies through a formal standardized training program.

Participate in education and quality improvement projects at the facility and area level as directed by Supervisor.

may occasionally be required to move up to 200 lbs of machines and equipment and lift chemical and water solutions of up to 30 lbs as high as 5 feet. Exposure to infectious and contagious diseases/materials. Rotates coverage with other licensed home therapy staff on an on‑call rotation, night, weekend, holiday or as defined by program needs. May require travel to training sites, other facilities and patient homes and perform essential functions in other locations with the same physical demands.

Supervision Assigned oversight of RNs, LPNs/LVNs, Patient Care Technicians and Home Therapies Care Team Assistants as a Team Leader or designated Nurse in Charge after meeting all required education, training and experience qualifications.

Education and Licensure Graduate of an accredited School of Nursing.

Experience and Required Skills Entry level for RNs with a minimum of 2 years of nephrology nursing experience within the last 2 years as an RN.

Minimum of 3 months experience in home dialysis therapies as an RN to qualify as a Team Leader.

Benefits Hourly Rate: $42.00 – $45.00 (dependent on location and qualifications).
