RN/LPN Nursing Positions - Weekends

Westlake Nursing and Rehabilitation Center
Licensed Practical Nurse (LPN)
Shift: 7:00 AM – 3:00 PM | 3:00 PM – 11:00 PM | 11:00 PM – 7:00 AM (Weekend and holiday availability may be required)

Administer medications, treatments, and therapies as prescribed.
Monitor and assess residents, reporting changes in condition to the RN and physician.
Supervise and support Certified Nursing Assistants (CNAs).
Maintain accurate and timely documentation in the electronic medical record (EMR).
Collaborate with physicians, therapists, social services, and other interdisciplinary team members.
Ensure compliance with CMS, state, OSHA, HIPAA, and facility policies.
Promote resident dignity, safety, and quality of life.
Respond promptly to resident needs and emergency situations.
Participate in care planning meetings, in-service education, and quality improvement initiatives. Current, active Licensed Practical Nurse (LPN) license in the State of Florida.
Graduate of an accredited Practical Nursing program.
Minimum of One-Year Long-term care or skilled nursing experience required.
Experience with electronic medical records (EMR) is required. Health, Dental, and Vision Insurance
