Health Caseworker

Overview Valir PACE provides integrated medical and social services to our most vulnerable seniors, helping to unlock their full potential. We are looking for team members who are energized by working in diverse teams toward our shared purpose. Those eager to drive groundbreaking innovation, and who will interact with those we serve and those who serve them in a spirit of understanding and compassion.
Responsibilities Coordinate all transitions of care, mental and psychosocial care.
Counsel participants on financial and advanced care planning options including advance directives, DNR, etc.
Advocate for participants and ensure participants’ wishes are enacted.
Qualifications Master’s degree in Social Work (MSW) from an accredited institution is required.
At least one year of experience working with the frail or elderly population is highly desirable.
Current valid Driver’s License, proof of auto liability coverage, and reliable transportation required.
Current LCSW certification is preferred.
