Responsibilities Deliver direct patient care in accordance with care plans and physician orders

Supervise, guide, and support CNAs to ensure high-quality care

Administer medications and treatments as prescribed

Monitor residents’ conditions and report changes promptly

Coordinate resident admissions, discharges, and transfers

Maintain accurate documentation in PointClickCare (PCC)

Foster resident-centered care and build strong relationships with families

Qualifications Active, unrestricted Illinois RN or LPN license

Current CPR certification

Strong communication and teamwork skills

Ability to provide compassionate, quality care in a fast-paced environment
