Description

Conduct evaluations in either inpatient or outpatient environments and develop personalized treatment plans
Provide physical therapy interventions focusing on mobility, strength, balance, and overall function
Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes
Educate patients and their families about therapeutic objectives, discharge strategies, and home exercise regimens
Document evaluations, interventions, and progress meticulously in line with institutional and regulatory standards
Engage in care team meetings and interdisciplinary discussions to guarantee cohesive care
Education
~ Doctor of Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications
Active state licensure as a Physical Therapist
CPR certification is preferred
Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Demonstrated experience is highly desirable
