Full-Time, Part-Time, and PRN opportunities
Flexible scheduling
Provide skilled nursing care in patients' homes under the supervision of a Registered Nurse.
Follow and carry out individualized Plans of Care established by the supervising RN.
Perform skilled nursing visits and complete accurate, timely clinical documentation.
Monitor patient status and promptly communicate changes to the supervising RN and healthcare team.
Educate patients and caregivers on medications, treatments, disease management, and self-care techniques.
Reinforce the Home Health Aide plan of care and collaborate with the interdisciplinary team to ensure quality patient outcomes.
Participate in case conferences, staff meetings, and quality improvement activities.
Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.

Graduate of an accredited Licensed Practical Nursing (LPN) program.
Current, unrestricted Ohio Licensed Practical Nurse license.
Minimum of one year of LPN experience required.
Home Health experience is preferred but not required.
Valid driver's license, reliable transportation, and auto insurance.
