Registered Nurse- Hospice Health

The Registered Nurse (Hospice) at Parentis Hospice and Health is responsible for providing compassionate and skilled nursing care to patients in the hospice setting. This role involves coordinating with interdisciplinary teams to ensure that patients receive comprehensive end-of-life care that respects their dignity and comfort while supporting their families through the process.

Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and interdisciplinary team members.

• Provide education and support to patients and their families regarding the disease process, symptom management, and end-of-life care options.
• Coordinate with physicians, social workers, and other healthcare professionals to ensure seamless delivery of care.
• Maintain accurate and timely documentation of patient care activities in compliance with regulatory and organizational standards.
• Participate in case conferences, team meetings, and quality improvement initiatives to enhance patient care and service delivery.
• Ensure adherence to hospice care policies, procedures, and ethical standards.
• Engage in continuous professional development to stay current with best practices in hospice and palliative care.

Bachelor of Science in Nursing (BSN) from an accredited institution

- Valid Registered Nurse (RN) license in the state of practice

Minimum of two years of clinical nursing experience, preferably in hospice or palliative care settings

• Experience in managing patient care plans and coordinating with interdisciplinary teams
• Proven track record of providing compassionate care to patients with terminal illnesses

Strong assessment and critical thinking skills to evaluate patient conditions and develop appropriate care plans

• Excellent communication skills to effectively interact with patients, families, and healthcare team members
• Proficiency in electronic medical record systems and documentation practices
• Ability to educate patients and families about disease processes, treatment options, and end-of-life care decisions
