Primary Care Physician - PACE / Geriatric Medicine

Position Overview

Seeking a Primary Care Physician to provide comprehensive, community-based healthcare services to seniors through a Program of All-Inclusive Care for the Elderly (PACE).

This full-time position focuses on delivering holistic, value-based care to vulnerable older adults while supporting their ability to remain in their homes and communities.

The physician will work collaboratively with an interdisciplinary team (IDT) to perform comprehensive patient assessments, develop personalized care plans, and coordinate medical services.

With a smaller patient caseload than traditional primary care settings, physicians can build meaningful, long-term patient relationships and focus on individualized healthcare needs.

Successful Completion of an ACGME-Accredited Residency Program - Required
Specialty Training in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics
Master's or Doctorate Degree - Listed as Required
Board Certification - Preferred
At Least 2 Years of Clinical Experience in the Specific Specialty - Required
Active, Unrestricted State Medical License - Required
State Controlled Dangerous Substances (CDS) Certification - Required
DEA Registration - Required
Eligibility to Enroll as a Provider for Medicare and Medi-Cal Participants - Required
CPR Certification Within 180 Days of Hire - Required
