A Referral Specialist contributes to AHF by conducting proactive outreach to clients by providing education and assisting with the enrollment of the Ryan White eligibility program. With the understanding of the Ryan White Program, you will work directly within the Healthcare Center to enroll newly diagnosed patients, patients lost to care, and clients currently in care by completing a screening eligibility assessment. A Referral Specialist enhances the reputation of AHF by making all necessary efforts to ensure clients are appropriately screened for Ryan White Eligibility under all other pertinent third-party benefits programs. With your skills of collaboration and engagement, you will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population. Your Eligibility assessments directly contribute to AHF's success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

To be successful as a Referral Specialist it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all that you do. AHF has a collaborative organizational structure where staff are accountable to multiple leaders.

The Referral Specialist will work as a part of a close-knit team to collaborate with the Care Management team and Healthcare Center staff so that every Ryan White client receive the support and Ryan White Eligibility assistance they require and access to available resources.

Licenses and Certifications: Associate's Degree/Bachelor's Degree in social science or human services-related field preferred.
