• The Sales Clinical Liaison acts as a patient advocate throughout the referral and admission process, ensuring patients are guided to the most appropriate level of care. This role requires a Registered Nurse (RN) or an individual with an advanced clinical degree.

Business Development & Outreach: Proactively grow the organization by identifying new opportunities, building strong referral relationships, and representing the company in the community.

Patient & Family Engagement: Act as a trusted, compassionate resource, educating patients and families about hospice services and guiding them through enrollment.

Strengthen partnerships with healthcare providers, ensure smooth communication between referral sources and clinical teams, and manage timely follow-ups and documentation.

Drive awareness and growth by educating stakeholders, analyzing referral trends, staying informed on market dynamics, and positioning the organization as a trusted leader in the community.

Effective relationship-building skills with referral sources, patients, and interdisciplinary teams

Customer service–oriented with strong active listening and professionalism

Working knowledge of medical terminology, healthcare delivery systems, and referral processes

Familiarity with electronic health records (EHR); hospice and palliative care knowledge preferred

Proficiency in Microsoft Office and ability to learn new systems quickly

Associate’s degree in nursing (ADN) or Bachelor of Science in Nursing (BSN)

Active, unrestricted Registered Nurse (RN) license in the state of practice.

Knowledge of healthcare referral processes and case management

Experience with hospice care and understanding of prognostic indicators

Two (2) years of clinical nursing experience, including case management in a clinical setting
