Hospital Liaison

We are seeking a dynamic and relationship-driven Hospital Liaison to join our growing team.
The Hospital Liaison serves as the primary connection between hospitals, physicians, case managers, discharge planners, and Eastern Healthcare Group's skilled nursing facilities. Develop and maintain strong relationships with hospital case managers, discharge planners, social workers, physicians, and healthcare professionals.
Conduct regular hospital visits to identify referral opportunities and support discharge planning efforts.
Coordinate smooth transitions for patients from hospitals to skilled nursing facilities.
Maintain referral tracking and provide regular activity and performance reports.
Participate in networking events, healthcare forums, and community outreach initiatives.
Bachelor's degree in Healthcare Administration, Marketing, Business, Nursing, Social Work, or related field preferred.
Minimum of 2 years of experience in healthcare sales, business development, admissions, case management, or hospital liaison roles.
Experience working with skilled nursing, rehabilitation, long-term care, home health, hospice, or hospital systems preferred.
Strong understanding of hospital discharge planning and post-acute care services.
Excellent communication, relationship-building, and presentation skills.
