What You'll Do & Bring As a Physical Therapist (PT) , you will play a vital role in helping patients regain strength, mobility, function, and independence while receiving care in their homes. You will perform comprehensive assessments, develop individualized plans of care, provide therapeutic interventions, supervise clinical support staff, and collaborate with the interdisciplinary care team to achieve optimal patient outcomes.
Develop, implement, and revise individualized Plans of Care in collaboration with physicians and the interdisciplinary care team.
Provide skilled physical therapy services, including therapeutic exercises, gait training, transfer training, balance training, fall prevention education, and functional mobility interventions.
Monitor patient progress, evaluate treatment effectiveness, and modify care plans as needed to achieve established goals.
Evaluate outcomes of care and coordinate appropriate discharge planning.
Provide wound care management as appropriate within the scope of practice.
Apply infection prevention and control principles to maintain a safe environment for patients and caregivers.
Maintain accurate, complete, and timely clinical documentation in accordance with agency policies and state and federal regulations.
Collaborate with physicians, nurses, therapists, home health aides, and other healthcare professionals to coordinate patient care.
Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (HHAs) as appropriate.
Participate in case conferences, quality improvement initiatives, mandatory continuing education, and in-service training programs.
What You Bring: Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
Current Texas Physical Therapist license in good standing.
Strong therapy skills based on accepted standards of physical therapy practice.
Ability to work independently while effectively collaborating with an interdisciplinary care team.
Ability to work a flexible schedule and travel within assigned service areas.
Commitment to delivering compassionate, patient-centered care.
Home health experience
Work Location & Travel Requirements: Home office location is MyCHN Home Health Services:4005 Technology Dr, Angleton, TX 77515, USA
This position requires regular travel to patient homes and other assigned locations throughout Brazoria, Galveston, and Harris Counties.
