Registered Nurse - On-Call (Hospice) Harmony Hospice of Ohio Schedule: Full-Time • On-Call • After-Hours & Weekends 7Days On/7 Days Off Locations:
Harmony Hospice of Ohio is committed to providing compassionate, patient-centered care to those we serve. We are seeking a dedicated and skilled Registered Nurse to join our On-Call team and support patients and families during after-hours, evenings, and weekends.

• The On-Call Registered Nurse provides after-hours and weekend support to patients and families, including visits, admissions, triage, and coordination of care. The RN responds promptly to calls, assesses needs, offers guidance per hospice standards, and collaborates with the interdisciplinary team to ensure continuity and quality of care.

Perform after-hours and weekend visits and admissions as needed.
Respond to incoming calls, triaging needs and directing calls to the DPCS, RN Clinical Supervisor/Manager, or RN Case Manager when appropriate.
Document all calls, conversations, and outcomes in the EMR system.
Receive start-of-shift reports and provide end-of-shift reports according to workflow.
Perform initial nursing assessments on new admissions.
Complete, coordinate, and revise patient Plans of Care in collaboration with the care team.
Educate families on patient condition, needs, and care expectations.
Make referrals to other hospice disciplines as needed and document rationale.
Must be a Registered Nurse with hospice experience preferred; telephone triage experience a plus.
Active RN license in good standing with the State of Ohio.
Strong understanding of hospice philosophy and the unique needs of terminally ill patients.
Flexible availability for after-hours, weekend, and on-call schedules.
Strong computer skills and ability to navigate EMR and software systems.
Must meet all hospice nursing competencies and maintain working knowledge of relevant software.
Ability to travel within the Harmony Hospice service area.
