Licensed Vocational Nurse (LVN) – PRN (As Needed)
Pay Rate: $35.00 per hour
Schedule: PRN with flexible scheduling based on facility needs

Clinical Care

Provide skilled nursing care, including medication administration, treatments, wound care, and monitoring resident conditions.
Deliver compassionate, resident-centered care that promotes comfort, dignity, and safety.
Assessment & Monitoring

Observe, assess, and document changes in residents' conditions.
Report significant findings promptly to the RN Supervisor and attending physicians.
Care Planning

Assist with implementing individualized care plans.
Support residents in achieving rehabilitation goals while maintaining the highest quality of care.
Supervise and support Certified Nursing Assistants (CNAs).
Work closely with physicians, therapists, and the interdisciplinary care team to ensure continuity of care.
Follow all federal, state, and facility policies, procedures, and regulatory requirements.
Current and active California LVN license required
Skilled nursing or post-acute care experience preferred
Compassionate, dependable, and team-oriented with a commitment to exceptional resident care
