The Providence Center helps adults, adolescents and children affected by psychiatric illnesses, emotional problems and addictions by providing treatment and supportive services within a community setting.

The Licensed Practical Nurse (LPN) supports client health and wellness by assessing client health needs, coordinating medical and psychiatric care, and administering medications. Working collaboratively with a multidisciplinary team, the LPN ensures the delivery of high-quality, person-centered services that promote recovery, stability, and improved health outcomes in both office-based and community-based settings.
Assist with collecting health information through observation, interviewing, record review, and team communication, and report findings to management.
Provide services to assigned caseload of clients, ensuring service intensity meets program standards, including required in-person contacts provided in both office- and community-based settings. Participate in ongoing client monitoring and contribute input to nursing assessments and care plans, implementing delegated interventions.
Identify urgent client concerns and assist with de-escalation, notifying the manager for full crisis assessment and intervention.
Respond calmly and professionally to emergencies, providing supportive measures within LPN scope.
Conduct home, community, or school visits to support client goals and daily functioning.
Update electronic medical record documentation for medications, lab tracking, and delegated clinical updates.
Assist with medication reconciliation by gathering information and communicating changes to the manager.
Encourage independence and assist with basic medical procedures, hygiene needs, and skill-building activities.
Uphold infection control practices, patient safety procedures, and agency policies.
Conduct home visits, school meetings, and community outreach to support clients in their natural environments.
Assist clients with basic medical procedures and hygiene needs. Ensure compliance with agency policies, ethical standards, and regulatory requirements.
Foster cooperative relationships with staff, clients, and community partners.
Support onboarding of new staff and student nurses as needed.
Bachelor's Degree preferred (diploma in nursing acceptable) Field of Study: Nursing
Experience working with psychiatric/mental health patients and substance use treatment preferred.
RI or Nursing Compact licensure required: Licensed Practical Nurse
Knowledge and understanding of the characteristics of individuals with mental health issues, their families, and family systems.
Skilled in crisis assessment, risk management, and de-escalation.
Ability to maintain professionalism and calm demeanor during emergencies.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Ability to compute rate, ratio, percent and draw and interpret bar graphs.
Knowledge of community resources.
Proficiency in documentation and electronic record-keeping systems, including ability to document in electronic medical records.
Capable of working collaboratively with community partners and in a team environment.
Ability to communicate effectively, both verbally and in writing, to convey information clearly and collaborate with multidisciplinary teams.
Ability to build rapport and establish trust with diverse individuals and community partners.
Ability to travel within the community with reliable transportation.
Bilingual preferred. For positions designated as bilingual (Bil), proficiency in the required language is mandatory, and the language assessment must be passed no later than 30 days of starting the role.
