Goals Of Care Registered Nurse

The Goals of Care Registered Nurse is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. The Goals of Care Registered Nurse serves as a consultant for partner organizations to complete Goals of Care conversations and documentation with the organization's members. S/he helps facilitate the completion of advanced care planning documents while applying the rules and regulations of state and federal regulatory agencies and other certified agencies. S/he serves as a clinical and educational role model and mentor, provides support in collaboration with the primary care team, and ensures compliance with practice standards and codes.

Performs Goals of Care conversations with members and/or families.

Educates patient and family to enhance understanding of disease processes and plan of care, including discussion of treatment options to ensure informed decision making.

Empowers patients to take control of their health care by encouraging appropriate goal-setting and discussions about advance care planning/completion of advance directives.

Partners with healthcare team to manage transitions of care to hospice or palliative care when appropriate.

Assists patients and families in identification of goals of treatment and ongoing plans of care and communicates these goals to the healthcare team.

Helps triage new consults to prioritize case load.

Prepares and maintains accurate patient records, charts, and documents.

Associate or Bachelor's degree in Nursing required.

Minimum of one (1) year experience in hospice and/or palliative care required.

Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.

Certifications, Licenses, and Registrations

Active and unencumbered Registered Nurse license in the state(s) of employment.

In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected.
