Monitor patient vital signs and report any changes to the nursing staff.
Document patient care and progress in compliance with company policies.
Collaborate with healthcare professionals to create personalized care plans.
Assist with mobility and physical therapy exercises as directed.
Participate in ongoing training and development to enhance caregiving skills.
High school diploma or equivalent; certification as a Home Health Aide preferred.
Previous experience in hospice or home health care is a plus.
Compassionate demeanor with a strong desire to help others.
Must be a Certified Nursing Assistant
