Physical Therapist Opportunity

Pride Health is hiring a Physical Therapist to support our client's home health program in Nutley, NJ (Coverage within Newark, Nutley, Belleville, and Bloomfield in New Jersey). This is a 6-month assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Job Details: Job Name: Physical Therapist Location: Nutley, NJ 07110 with Travel within Newark, Nutley, Belleville, and Bloomfield in New Jersey Job Type: Local Contract Duration: 6+ months Contract (Possibilities of extension) Pay rate: $50 - 56/hr. Reimbursement provided (from second patient to last patient)

Perform Start of Care (SOC) visits, completing initial assessments and establishing individualized Plans of Care (POC) in accordance with physician orders.

• Conduct thorough physical therapy evaluations assessing strength, mobility, balance, pain, and functional limitations.
• Provide skilled therapy interventions during routine and follow-up (revisit) appointments.
• Complete timely and accurate clinical documentation in compliance with regulatory and organizational standards.
• Coordinate care with physicians, interdisciplinary team members, and caregivers to ensure optimal patient outcomes.
• Conduct discharge visits, including final assessments, patient education, and transition planning.

Graduate of an accredited Physical Therapy program (bachelor's or higher).

Active, unrestricted Physical Therapist (PT) license in New Jersey.

Minimum 1 year of clinical experience (home health experience preferred).
