Licensed Vocational Nurse (LVN) - Part-Time
Pay Rate: $40.00 per hour

San Francisco Post-Acute is seeking a dependable, positive, and patient-focused Licensed Vocational Nurse (LVN) to join our skilled nursing team. This flexible position is ideal for professionals who thrive in a dynamic healthcare environment and are committed to providing exceptional resident care.
As an LVN, you will play an essential role in supporting resident health and well-being while collaborating with nurses, physicians, and interdisciplinary care teams. This position requires flexibility to work various shifts based on facility needs.
Part-Time Flexible position

Administer medications and treatments in accordance with physician orders and facility policies.
Maintain accurate and timely resident documentation and medical records.
Conduct daily rounds and monitor resident conditions.
Communicate changes in resident status to the nursing and healthcare team.
Assist with admissions, transfers, and discharge planning.
Support quality care initiatives and ensure compliance with regulatory standards.
Active California Licensed Vocational Nurse (LVN) license required.
Skilled nursing or post-acute care experience preferred but not required.
