Licensed Practical Nurse (LPN) – Home HealthFull-Time | Part-Time | PRNExcellent Pay + Paid Mileage + Flexible SchedulingDual MI and OH license preferred but not required.Freedom at Home is a Veteran-Owned Home Health Agency and we're looking for compassionate, motivated Licensed Practical Nurses (LPNs) to join our growing team.If you're looking for a company that values its clinicians, promotes work-life balance, and provides the support you need to deliver exceptional patient care—we'd love to meet you.What We OfferCompetitive payFull-Time, Part-Time, and PRN opportunitiesPaid mileageFlexible schedulingSupportive leadership teamPositive, clinician-focused cultureOpportunities for growth as we expandThe chance to make a meaningful difference in patients' lives every dayResponsibilitiesProvide skilled nursing care in patients' homes under the supervision of a Registered Nurse.Follow and carry out individualized Plans of Care established by the supervising RN.Perform skilled nursing visits and complete accurate, timely clinical documentation.Monitor patient status and promptly communicate changes to the supervising RN and healthcare team.Educate patients and caregivers on medications, treatments, disease management, and self-care techniques.Reinforce the Home Health Aide plan of care and collaborate with the interdisciplinary team to ensure quality patient outcomes.Participate in case conferences, staff meetings, and quality improvement activities.Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.QualificationsGraduate of an accredited Licensed Practical Nursing (LPN) program.Current, unrestricted Ohio Licensed Practical Nurse license.Minimum of one year of LPN experience required.Home Health experience is preferred but not required.Valid driver's license, reliable transportation, and auto insurance.At Freedom at Home, our clinicians are our greatest asset.
