Role
Full-time Physical Therapist

Responsibilities
Provide compassionate, evidence-based rehabilitation services to seniors and medically complex residents.

Assess residents' functional status

Design and implement individualized treatment plans

Deliver interventions to enhance strength, mobility, balance, and safety

Collaborate with an interdisciplinary team

Educate residents and families about care plans

Document progress and outcomes

Support safe discharges and transition of care

Qualifications & Skills

Current licensure as a Physical Therapist

Strong assessment and treatment planning skills

Excellent communication and collaboration abilities

Ability to work compassionately with elderly and medically complex populations

Knowledge of resident-centered, dignity-focused care practices
