You will be responsible for assessing patients' needs, coordinating admissions, and ensuring a smooth transition into hospice services. Your focus will be on delivering high-quality, personalized care while respecting patients' dignity and autonomy during this sensitive time.Essential job functions/responsibilitiesConduct comprehensive assessments to determine patients' eligibility for hospice care in consultation with the attending physician and medical director.Educate patients and families about hospice services, treatment options, and end-of-life planning.Develop individualized care plans based on patients' goals, preferences, and clinical assessments.Collaborate with physicians, social workers, chaplains, and other team members to optimize patient care.Monitor patients' conditions and response to treatment, adjusting care plans as needed.Ensure compliance with regulatory standards and hospice policies in patient care delivery.Ordering and coordinating delivery of approved medications, medical supplies, and durable medical equipment to meet the needs of the patient and family.Position qualificationsGraduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting and one (1) to two (2) years of recent experience in hospice or palliative care.Current registered nurse licensure in the state of Georgia and CPR certification.Excellent observation, verbal and written communication skills, critical thinking and problem-solving, basic math, and nursing skills per competency checklist.Able to lift supplies and equipment. Visual acuity and hearing to perform required nursing skills.Proficiency in computer applications, including electronic medical records systems and Microsoft Office Suite.
