Responsibilities

Provide professional nursing care to patients via direct and/or telephone contact, following established standards and practices.

Coordinate with other care team members to ensure seamless care delivery, maximal coordination of efforts, and active patient participation in planning and care.

Assist physicians or other providers with clinical procedures and perform telephone triage as needed.

Provide direction to Licensed Practical Nurses, clerical assistants, medical assistants, and other non-licensed personnel as applicable.

Demonstrate clinical competence in direct patient care, considering developmental differences among patients.

Demonstrate knowledge of common disease processes and treatments, including pertinent diagnostics and interventions, with focus on preventative health measures for the population served.

Use evidence-based approaches with patients, families, and caregivers to support self-management and health-promoting behavior change.

Collaborate with patients and the healthcare team to establish goals and priorities and ensure the plan of care reflects patient needs and preferences.

Contribute to ongoing assessment and management of health needs of assigned patient populations using population-specific knowledge and clinical expertise.

Perform telephone triage with emphasis on assessment, problem/priority identification, and decision-making.

Administer prescribed medications correctly, with knowledge of dosage, route, site, and potential side effects.

Provide accurate, up-to-date clinical information to patients consistent with professional nursing scope and practice standards, policies, and protocols.

Perform authorized technical procedures competently (e.g., EKG, phlebotomy, starting/monitoring IVs, selected laboratory tests) as required in the clinical practice area.

Seek validation or guidance from physicians or nurse colleagues when necessary.

Qualifications

Required Qualifications:

Graduate of an accredited school of nursing.

Registered Nurse licensed to practice in New York State (NYS) with current NYS registration and in good standing.

Preferred Qualifications:

Bachelor of Science in Nursing (B.S.N.).

Current Basic Life Support (BLS) certification.

2+ years of recent pertinent professional nursing experience in a hospital, medical office, or other clinical setting.

1+ years of experience as a registered nurse or 2+ years as a licensed practical nurse in a pertinent practice area.
