SUMMARY Plan, organize, develop and implement a progressive therapy program geared to improvement of resident’s physical functions, psychological, social and economic well being. Follow corporate guidelines, objectives and policies to maintain standards of treatment consistent with legal and accrediting requirements.

ESSENTIAL DUTIES AND RESPONSIBILITIES ASSESSMENT Upon receiving SLP orders, review client’s history and complete a thorough assessment, including precautions.

Upon receiving SLP screening request, review client’s current situation and screen individuals for intervention.

Communicate assessment/screening results to client and appropriate family and/or staff.

Complete required assessment/screening documentation.

PLANNING Identify and prioritize needs based on assessment and establish an appropriate plan with goals.

Communicate treatment plan and goals to client and family.

Collaborate with health care team members to begin planning for appropriate and timely discharge or transfer.

IMPLEMENTATION Demonstrate ability to prioritize treatment procedures according to client’s needs and MD requests.

Implement interventions according to plan of care.

Communicate purpose of treatment procedures to client/family according to client’s level of understanding and modify plan accordingly.

Communicate ongoing treatment issues/plan to interdisciplinary health care team members.

Complete required documentation regarding daily intervention.

EVALUATION Review and evaluate effectiveness of interventions toward achieving client outcomes.

Document appropriate reassessment and changes in plan of care.

Communicate significant changes in intervention to client/family and health care team members.

Complete required documentation for discharge.

PROGRAM DEVELOPMENT Under the supervision of the Lead Therapist and/or the Director of Rehab, develop and implement new programs to enhance effectiveness of Rehab Program; review existing programs on an ongoing basis.

Develop, implement and supervise student practicum under the supervision of the Lead Therapist and/or Director of Rehab.

Develop and present orientation/inservice training for facility employees and community participants.

Assist in developing and maintaining Rehab policies and procedures.

PROFESSIONAL SELF-GROWTH Attend at least one continuing education course annually and present information to staff within four weeks of attending course.

Attend at least three facility inservices annually including all mandatory inservices.

Participate in organizational committees.

OTHER DUTIES Perform other related duties as assigned.

Support and participate in changes and suggestions made by the Director of Rehab and/or Lead Therapist.

SUPERVISORY RESPONSIBILITIES Supervise student practicum and other staff as assigned.

EDUCATION AND/OR EXPERIENCE Master's degree (M. A.) or equivalent; or four to ten years related experience and/or training; or equivalent combination of education and experience.
