The ONN serves as a dedicated clinical partner for members navigating a cancer diagnosis, providing personalized education, care coordination, and ongoing support throughout the treatment and survivorship journey.

Candidates must be Dallas area based with the ability to work in-house up to five days per week depending on business need. Remote days require a private, secure home workspace free from other employment, caregiving, or school obligations during working hours.

Conduct comprehensive oncology intake assessments to establish member needs, goals, and care priorities.

Coordinate multidisciplinary care by facilitating communication between members, treating providers, and the broader LCC team.

Identify and address barriers to care including psychosocial, financial, and logistical needs; Complete accurate, timely documentation of all member interactions and clinical activities within Lantern's care management platform.

Complete all outreach, follow-ups, and care coordination tasks within defined timeframes.

Participate in call auditing processes and consistently apply feedback to maintain and improve quality of care delivery.

Collaborate closely with the Lead ONN, LCSWs, Medical Records Specialists, Care Advocates, and cross-functional partners.

Contribute to team meetings, case reviews, and quality improvement initiatives.

Bachelor's degree in Nursing (BSN)

Active RN license in a compact state with unencumbered ability to be licensed in all 50 states

Minimum 3 years of oncology nursing experience

Active ONCC certification in Oncology Nursing (OCN) with unencumbered ability to maintain certification throughout employment

Proficiency in Microsoft Outlook and Microsoft Office Suite

Demonstrate strong clinical judgment and deep knowledge of oncology diagnosis, treatment, and survivorship care

Be detail-oriented and disciplined in documentation, follow-through, and quality standards

Maintain a full, balanced caseload while meeting all productivity and quality expectations

Demonstrate a genuine commitment to equitable, member-centered care across a diverse oncology population
