Registered Nurse Position
Responsible for planning and carrying out individualized nursing care based upon accurate assessment and evaluation of the actions performed, according to the treatment plan prescribed by the provider. Provide support and assistance in other departments/sites as needed or directed.
Essential duties and responsibilities include managing patient flow, assessing vital signs, establishing personal rapport with potential and actual patients, developing and implementing educational plans, checking for and/or obtaining signed informed consent forms, assisting with routine examinations and procedures, assessing and referring patients for support services, administering medications, authorizing medication refills, maintaining required tracking within the health center system, functioning within the legal and ethical parameters of nursing care, educating staff members on current knowledge, identifying problem areas, assisting with maintaining quality improvement and clinical performance metrics, recommending process improvement/changes, knowing infection control procedures, training new hires/students, working assigned clinical inbox, maintaining and tracking supplies and equipment, and performing other duties as assigned.
Knowledge, skills, and abilities include being professional in appearance and behavior, being customer service oriented and focused, being dependable and punctual, motivating staff, possessing a positive attitude, having knowledge of current trends in nursing, being respectful of cultural and religious practices, working effectively independently and within a team, and having standard clerical dexterity.
Education and experience require a degree from an accredited School of Nursing, a current license to practice as a Registered Nurse in the State of Illinois, and maintaining BLS certification.

Medical home statement includes understanding expectations and responsibilities of the Patient Centered Medical Home, understanding role, completing relevant duties, and maintaining the highest level of proficiency within scope as part of this team-based approach in the patient care continuum.
Physical demands include being able to lift up to 50 pounds, assist in patient lifts/transfers utilizing proper body mechanics, stand for extended periods, walk on a regular basis, and sit for extended periods of time.
Work schedule includes normal work hours from Monday through Friday during dayshift unless otherwise specified.
