The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and - through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides - coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Senior Care Partners PACE is seeking a Physical Therapist - Float to provide compassionate, participant-centered care across our Kalamazoo and Calhoun County locations. This is an opportunity to use your clinical expertise in a dynamic setting where you’ll help older adults maintain mobility, independence, safety, and quality of life—both at our PACE centers and in the community.

What You’ll Do

Evaluate participants’ mobility, physical abilities, and rehabilitation needs.

Develop and implement individualized physical therapy plans of care.

Provide therapy through therapeutic exercise, modalities, manual therapy, and other evidence-based interventions.

Float between Kalamazoo and Calhoun counties based on participant and program needs.

Collaborate closely with the Interdisciplinary Team (IDT) to coordinate participant-centered care.

Assess and recommend adaptive and durable medical equipment and support equipment transitions.

Educate participants, families, and caregivers to promote safety and independence.

Support physical wellness programming and help develop innovative programs for participants.

Provide guidance and mentorship to PTAs, rehab tech PCAs, and PT students when applicable.

Support participants transitioning home from hospitals, skilled nursing, assisted living, and other care settings.

Participate in quality improvement initiatives and promote exceptional customer service.

What You Bring

Doctorate in Physical Therapy (DPT), unless grandfathered under applicable requirements.

Graduate of an approved physical therapy program.

Current, unencumbered Michigan Physical Therapist license.

At least 1 year of experience working with frail, elderly, or long-term care populations.

Strong communication, organization, problem-solving, and collaboration skills.

Working knowledge of Microsoft Office.

Current AHA CPR certification or willingness to obtain certification.

Work Environment

Has a visible presence while working in the center and is subject to frequent interruptions.

Communicates with staff, participants, caregivers, the board of Senior Care Partners PACE and other organizations as necessary.

Works beyond normal working hours and on weekends and holidays when necessary.

May be subject to exposure and handling of infectious waste, diseases, conditions, or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or the Hepatitis B virus.
