Reports to: Nursing Supervisor

FLSA: Non-exempt (hourly)

Schedule: Full Time; 12 hour shifts – 3 days one week, 4 days the next, with an every‑other‑weekend rotation required.

The Hoy Center takes a holistic approach to skilled nursing care and aging. Our person‑centric living aligns the delivery of care with individual needs and preferences. Residents live in home‑like, comfortable Lighthouses, each with a family room and e‑concept kitchen, and enjoy beautiful views and ocean breezes on Observation Decks.

Perform blood glucose monitoring, inhalation treatments, and other prescribed medical treatments/diagnostic tests.

Maintain medication and resident records, responsible for electronic input of resident’s data in the EHR system.

Transmit medication refill requests and other necessary documentation via scans and/or faxes per policy.

Communicate and coordinate care with residents and their families.

Document and lead in the documentation of resident care.

Identify, report, and document incidents and accidents; Review, transcribe, and implement physician orders.

Communicate with resident’s physicians as directed.

Implement and ensure staff adherence to infection control measures.

Lead CNAs (Certified Nursing Assistants).

Support and participate in community and departmental quality assurance programs.

Ensure unit practices are consistent with regulatory requirements.

Maintain positive working relationships between nursing staff and other departments.

Compassionate, patient‑centered approach to care.

Must be able to stand or walk for an extended period and push a wheeled medication cart through the assigned unit.

Active, unencumbered LPN license in the state of Virginia (or compact).

At least one year of nursing experience, including one year in a long‑term care facility.

Experience with Microsoft and Google applications.

Compensation: $33.00/Hourly + (based on relevant experience)
