Your Role: Home Health LVN
As a Licensed Vocational Nurse (LVN) in Home Health, you will deliver skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). You'll play a vital role in supporting patients through rehabilitation, chronic condition management, and senior care services.
Provide skilled home health nursing care based on individualized care plans
Support senior care and rehab patients in home and community settings
Document patient visits, treatments, and progress in EMR systems
Collaborate with physicians, RNs, families, and interdisciplinary teams
Assist patients with chronic conditions, recovery, and dementia care
Ensure compliance with California home health regulations
Promote patient safety, independence, and overall well-being
Active California Licensed Vocational Nurse (LVN) license in good standing
Graduate of an accredited vocational nursing program
Minimum 1 year of nursing experience (home health preferred)
Experience in senior care, rehab, or community care preferred
Valid driver's license, reliable transportation, and auto insurance
Experience with electronic medical records (EMR systems)
Flexible Per Diem LVN schedule Work locally within Los Angeles County Experience in home health, senior care, rehab, and community care Meaningful, patient-centered work with aging and rehab populations Supportive, mission-driven healthcare team
