Description

Conduct assessments of patients in both inpatient and outpatient contexts, crafting tailored treatment plans
Implement physical therapy interventions aimed at enhancing mobility, building strength, and improving overall functionality
Work collaboratively with nurses, physicians, case managers, and other therapy professionals to achieve optimal outcomes for patients
Guide patients and their families through therapeutic objectives, discharge preparations, and at-home exercise regimens
Keep thorough documentation of evaluations, treatments, and patient progress, adhering to regulatory guidelines
Engage in care team discussions and interdisciplinary meetings to ensure seamless care coordination
Education
~ Doctorate in Physical Therapy (DPT) or a related degree from an accredited institution

Certifications
Valid state licensure as a Physical Therapist
CPR certification preferred
Specialty certifications (e.g., geriatric, neurological) are advantageous but not mandatory
Skills
Prior experience in a clinical setting is strongly preferred
Experience treating patients with complex medical conditions and mobility challenges
