Demonstrates excellent customer service skills with patients, families, customers, and co-workers. Must be resourceful in overcoming hospice barriers with patients and caregivers. Responsible for obtaining referral orders and patient records.Unrestricted LPN or RN license. Minimum of 2 years of nursing experience in home health, or healthcare facility. Must be able to deal with human suffering while maintaining professional boundaries. Required to be flexible and change schedule at a moment's notice to meet patient needs.
