Registered Nurse | Hospice Care | Evening Shift

Hospice Registered Nurse
Supplemental Health Care is hiring compassionate and experienced Hospice RNs to provide patient-centered care for individuals and families in Medford, Oregon, and surrounding areas.
Active, unencumbered RN license in the state of assignment

1+ year of recent experience (within last 3 years) in hospice, home health, palliative care, or acute care
Strong knowledge of symptom management, pain control, and hospice documentation (CMS CoPs, Medicare Hospice Benefit)
current TB test and immunizations
Reliable transportation and valid auto insurance
Flexibility for travel, occasional evenings, weekends, and on-call shifts

Hospice RN Contract Details:

$Nurse will have a designated territory of coverage
Travel time included in scheduling
All nurses on contract with SHC are paid weekly

Perform comprehensive assessments and develop individualized hospice care plans
Deliver skilled nursing care, symptom management, and emotional support
Educate patients and families on hospice philosophy and comfort care practices
Collaborate with the interdisciplinary team to review and adjust care plans
Maintain accurate documentation in compliance with hospice standards
