Schedule: Monday - Friday, 8:15AM - 5:00PM with on-call requirements
The Licensed Practical Nurse (LPN) I provides comprehensive skilled care as ordered by the attending physician by following the order sets created by the RNCM. The LPN I educates patients and their family members and ensures the safety of the patient. The LPN I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The LPN I is responsible for all practices and duties within the scope of practice as outlined by the state.
Provides skilled nursing care in a patient's home as ordered by the attending physician
Performs care by following order sets and goals made by RN
Administers medication as prescribed by the physician
Incorporates patient and family in development of plan of care
Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions
Documents all aspects of treatment, assessments, and patient education
Maintains active LPN license in state of employment
Understands and practices agency policies and procedures
Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines. Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.
Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
Minimum of one year of experience as a LPN in an acute care setting is required; Home care experience is desired.
Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.
Knowledgeable on nursing best practices.
Ability to make appropriate nursing judgments.
Valid driver's license and auto insurance with your name as a listed driver.
Licensed Practical Nurse with current license in the state of employment

Transitional work is available (although some jobs may require specific training) such as: answering phones, filing, computer work/data entry, attending training/continuing education, reviewing safety manuals & other educational media, and assisting or performing other tasks within restrictions assigned.
