Reports to: Director of Clinical Nutrition

Van Duyn Center for Rehabilitation and Nursing is a 513-bed skilled Nursing facility atop Onondaga Hill in Syracuse, NY. Coordinate nutritional services for residents
Complete and document nutrition assessments
Participate in the resident care planning process
Provide nutritional education and counseling to residents and families
New York State Registered Dietitian
Rehab experience is a plus!
Excellent Customer Service skills
