Job Description Medical Social Worker (MSW) PRN Make a Difference Beyond the Clinical Visit A patient’s well-being depends on more than medical care. Your support can help patients and families access resources, navigate challenges, and remain safely at home.

Elara Caring is hiring a compassionate PRN Medical Social Worker to support home health patients and families throughout Portland and surrounding southern Maine communities.

Work independently in the community

Help patients access essential services and resources

What You’ll Do Assess patients’ psychosocial, emotional, financial, and environmental needs

Complete initial social work evaluations and follow-up visits

Connect patients and caregivers with community programs and financial resources

Help patients and families navigate healthcare and community services

Participate in developing and updating individualized Plans of Care

Collaborate with physicians, nurses, therapists, and other care team members

Support discharge planning and continuity of care

Communicate concerns or changes affecting patient well-being

What You’ll Bring Master’s or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education

At least one year of social work experience in a healthcare setting

Knowledge of community programs and available patient resources

Willingness to travel throughout the assigned service area with 50% travel required

Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations

Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
