Pro Active Physio Clinic is seeking a dedicated and passionate Physical Therapist (PT) to join our team full-time in Simi Valley, California. The successful candidate will play a crucial role in evaluating patients and designing comprehensive treatment plans, ensuring quality care and positive health outcomes. This position is primarily onsite in home care settings, embracing the dynamics of today's healthcare landscape. As our clinic provides therapies directly in the home environments of our clients, flexibility is key. The normal working hours are Monday through Friday from 8 AM to 3 PM, facilitating a balanced work-life integration. Conduct thorough Physical Therapy evaluations for pediatric and adult patients, assessing their unique needs and conditions.
Regularly reassess and document clinical progress, adjusting treatment plans as necessary to optimize patient recovery.
Document all patient interactions comprehensively, including SOAP notes, progress summaries, and care plans, utilizing the clinic's electronic health record system efficiently.
Educate patients and their families about treatment processes, rehabilitation methods, and the potential challenges they may experience along the way.
Achieve established utilization goals and contribute to the business objectives of the clinic.
Collaborate with multidisciplinary teams within Pro Active Physio Clinic to provide cohesive care for patients accessing multiple services.

By employing both in-person and telehealth services, you will ensure accessibility and convenience in patient care, creating an inclusive environment that prioritizes patient outcomes.
A Master's degree or Doctor of Physical Therapy (DPT) from an accredited physical therapy program is required.
Current and valid licensure to practice Physical Therapy in California.
Demonstrated experience in performing Physical Therapy evaluations and treatments, both in-person and remotely when clinically appropriate.
Flexibility and adaptability to navigate the dynamic healthcare environment effectively.
Strong collaboration and teamwork skills, complemented by a customer-focused approach to care.
